Return pages 4 and 5 to the Office of Theses and Dissertations, along with a copy of your title page and abstract. You are not required to provide a hard copy of the dissertation to ProQuest. The copyright registration is optional, and you will still own the copyright even if you choose not to register it.
If you wish to order copies through ProQuest page 7; also optionalkeep in mind that copies are made from the microfilm and will not arrive until approximately 10 months after your graduation if you have not chosen to restrict access.
In the case of restricted access, ProQuest will not be able to produce your copies until the restriction is lifted.
